How to beat: Plague maiden
When provoked, the pesta summons clouds of plague-carrying insects that hunt Geralt across the area. Burn the swarms with Igni, cut them with the silver sword or push them back with Aard. Yrden holds the maiden herself in place so you can strike her, but staying near her also leaves you exposed to the insects. Coat your silver sword with Specter oil.
About
The plague maiden, also called a pesta, is a wraith that roams lands ravaged by epidemics in the form of a sick woman with rotting flesh, followed by swarms of rats. No one knows whether she brings disease or is merely drawn to it, but she clearly delights in human suffering. In The Witcher 3 she is met on Fyke Isle.
